Templates

  • IRB Statement – This statement should be used in recruitment of participants (emails, social media, flyers, etc.). This statement may be added to the bottom of the recruitment post, flyer, email, etc.

  • Informed Consent Template – This template should be used when the IRB requires documentation of informed consent for research participants that are 18 years of age or older and have the capacity to provide consent.

  • Parental Consent Template – This template should be used when the IRB requires documentation of parent/guardian consent for their child/ward that are under 18 years of age to participate in a research study.

  • Student/Child Assent Template - This assent template is should be modified as necessary so that it can be easily understood by the participant population. This form is provided to the student/child participant only after the parent/guardian has given consent for their child/ward to participate. Student/child’s signature is not required.

  • Research Statement (Survey) – This template should be used when conducting an online or paper survey (Non-sensitive questions). No identifiable information will be collected and shared in a public presentation and/or publication.

  • Research Statement (Interview) – This template should be used when conducting an interview. No identifiable information will be collected and shared in a public presentation and/or publication.

  • Research Statement (Focus Group) – This template should be used when conducting a focus group. No identifiable information will be collected and shared in a public presentation and/or publication.

  • Research Statement (Interview - recorded) – This template should be used when recording an interview for the sole purpose of accurately capture participant concerns, opinions, and ideas. Once the recordings have been transcribed, the tapes must be deleted from all devices.

  • Research Statement (Focus Group - recorded) – This template should be used when conducting a focus group and a recording will be made to accurately capture participants concerns, opinions, and ideas. Once the recording has been transcribed, the recordings must be deleted from all devices.
